Temporary circulatory support with surgically implanted microaxial pumps in postcardiotomy cardiogenic shock following coronary artery bypass surgery

چکیده
ObjectivesPatients with ischemic cardiomyopathy undergoing coronary artery bypass grafting (CABG) surgery may develop postcardiotomy cardiogenic shock. In these cases, implantation of an Impella 5.0 or 5.5 microaxial pump offers full hemodynamic support while simultaneously unloading the left ventricle.MethodsPreoperative, perioperative, and postoperative data all patients receiving after CABG between September 2017 October 2022 were retrospectively collected. Cohort built-up was performed according to timing implantation, either simultaneous during delayed.ResultsA total n = 42 received support, whom 27 underwent 15 delayed therapy. Preoperative ventricular ejection fraction similarly low in both groups (26.7 ± 0.7% vs 24.8 11.3%; P .32). cohort, a median 1 (1; 2) days surgery. Survival 30 (75.6% 47.6%, .04) year (69.4% 29.8%, .03) better cohort implantation.ConclusionsThe combined advantages LV pumps lead favorable survival failure following Early initial shows trend toward more as compared support.

Acquired von Willebrand syndrome in cardiogenic shock patients on mechanical circulatory microaxial pump support
Early use of mechanical circulatory support, e.g. veno-arterial extracorporeal membrane oxygenation (ECMO) or left ventricular unloading by microaxial pump in refractory cardiogenic shock is recommended in current guidelines. Development of acquired von Willebrand Syndrome (AVWS) in patients with left ventricular assist devices (LVADs) and ECMO has been reported. There is an increasing number o...
متن کاملMechanical circulatory support in acute cardiogenic shock
Cardiogenic shock complicates about 5% to 8% of all admissions for acute myocardial infarction, and despite advancement in treatment over the past 50 years, mortality remains unacceptably high. Management with vasoactive agents after revascularization can have its limitations and thus mechanical circulatory support is often initiated. Intra-aortic balloon pumps (IABPs) are the devices most comm...
متن کاملAssessment of cognitive impairments following coronary artery bypass surgery with emphasis on working memory performance
Background: Previous researches have provided contradictory results about on working memory performance after the coronary artery bypass graft (CABG). In addition, studies have focused on the elderly community. For this reason and with regard to the importance and direct effects of working memory on the quality of life human. This study was designed to compare working memory of young CABG patie...
متن کاملExtracorporeal membrane oxygenation support for adult postcardiotomy cardiogenic shock.
BACKGROUND Postcardiotomy cardiogenic shock occasionally develops in patients who have undergone cardiac procedures. We report our experience using extracorporeal membrane oxygenation (ECMO) in adult patients with postcardiotomy cardiogenic shock, and analyze the factors that affected outcomes for these ECMO patients. METHODS We retrospectively reviewed the medical records of ECMO patients. ...
متن کاملPostcardiotomy VA-ECMO for refractory cardiogenic shock
Postcardiotomy cardiogenic shock (PCCS) is a rare but catastrophic syndrome that can occur following separation from cardiopulmonary bypass or at any time during the immediate postoperative course. The management of PCCS varies between clinicians, institutions and countries. The available evidence to guide this practice is limited. In their systematic review and meta-analysis, Khorsandi and col...
